Overview

[18F]-AlF-NOTA-pHLIP is a diagnostic PET imaging radiopharmaceutical designed to target the acidic extracellular environment of tumors. It consists of a pH (low) insertion peptide (pHLIP) conjugated to a NOTA chelator, which is labeled with an aluminum [18F]fluoride ([18F]AlF) complex. The pHLIP peptide is a water-soluble polypeptide that undergoes a conformational change and inserts into the cell membrane specifically under acidic conditions (pH < 7.0), which are characteristic of the tumor microenvironment due to increased glycolytic metabolism. This mechanism allows for the delivery and retention of the PET isotope at the tumor site, providing a metabolic-based imaging alternative to receptor-targeted tracers. It has been investigated preclinically for imaging various cancers, including breast, prostate, and melanoma.
